dc.description.abstractO índice de extração de oxigênio (IEO2) cerebral tem sido usado em pacientes com traumatismo craniano como método de avaliação da hipoperfusão. A proposta deste estudo foi medir pela primeira vez este índice em pacientes submetidos à endarterectomia de carótida. Trinta e nove procedimentos foram estudados. Os pacientes foram randomizados em dois grupos: grupo 1 em que o shunt foi randomicamente utilizado, e o grupo 2, em que não se fez a utilização deste. Pacientes com pressão retrógrada a partir da carótida interna abaixo de 50% da pressão arterial média constituíram um terceiro grupo. Os parâmetros avaliados foram a saturação venosa jugular de oxigênio, glicemia, lactato e pH venosos, temperatura, hemoglobina e, finalmente, o IEO2, através de um cateter posicionado no bulbo superior da veia jugular interna para coleta de amostras sangüíneas em três momentos: antes do pinçamento, imediatamente antes de liberar as pinças e 5 minutos após restabelecido o fluxo carotídeo. O grupo 1 apresentou valores de IEO2 de 0,20, 0,21 e 0,20, nos três momentos avaliados; no grupo 2, os valores foram de 0,18, 0,23 e 0,21, respectivamente. Não foi encontrada significância estatística nesta comparação nem em relação às outras variáveis analisadas. Os grupos eram semelhantes quanto à idade, sexo, pressão arterial média, hemoglobina, saturacão arterial de oxigênio, pH, pressões arteriais de oxigênio e dióxido de carbono, além da glicemia.Os níveis críticos de hipoperfusão cerebral durante a endarterectomia de carótida a partir do IEO2 não puderam ser determinadas por este estudo. Não houve diferença estatisticamente significante entre os grupos com relação a este índice, sendo o IEO2 um método pouco prático ou seguro para estimar a isquemia cerebral durante a endarterectomia de carótida.pt_BR
dc.description.abstractCerebral oxygen extraction index (O2EI) have been used in patients with cranial trauma as a method to evaluate cerebral hypoperfusion. The purpose of the study was to measure, for the first time, the O2EI in patients submitted to carotid endarterectomy. Thirty eight patients submitted to carotid endarterectomy were studied. Patientes were randomized in two groups: in group I were those in which no intraluminal shunt was inserted as a cerebral protection, and group II included patients submitted to simple cross-clamping. Patients with a stump back pressure below 50% of systemic mean arterial pressure formed a third group. The following parameters were measured: jugular venous oxygen saturation, glocoses, lactate, pH, temperature and hemoglobine, and finally, the O2EI. A catheter was inserted in the internal jugular vein in the superior bulb. Blood samples were colected in three moments: before cross-clamping, immeadiately before releasing the clamps and 5 minutes after the liberation of the flow to internal carotid artery. The O2EI was 0,20, 0,21 and 0,20 in the shunted group and 0,18, 0,23 and 0,21 in the group submitted to simple cross-clamping in the three moments described above. There was no statistical significance difference regarding jugular venous oxygen saturation, glucoses, lactate, pH, temperature and hemoglobine. The two groups were similar regarding gender, age, clinical stratification, arterial mean pressure, hemoglobine, arterial oxygen saturation, pH, arterial O2 and CO2 pressure, and glucoses. The results achived in the study are not statistical significant. The critical point of O2EI related to cerebral hypoperfusion can not be determined during carotid endarterectomy in the present study. There was no statistical difference between the shunted group versus not shunted. The O2EI is not a practical or secure method to estimate cerebral ischemia during surgery.en
