dc.description.abstractO presente estudo investigou a experiência da paternidade no contexto da prematuridade desde o nascimento e internação do bebê em unidade de tratamento intensivo neonatal (UTINeo) até o 3º mês após a sua alta hospitalar. Buscou ainda investigar retrospectivamente as expectativas e sentimentos do pai durante a gestação. Participaram três pais de bebês nascidos pré-termo, com idades entre 27 e 45 anos e que residiam com a mãe do bebê. Dois participantes estavam sendo pais pela primeira vez, e um deles estava tendo um terceiro filho. Os bebês apresentavam diferentes características clínicas ao nascer, incluindo um bebê nascido extremamente pré-termo e com extremo baixo peso, um bebê nascido muito pré-termo e com muito baixo peso e um bebê nascido moderadamente pré-termo e com baixo peso. Foi utilizado um delineamento longitudinal de estudo de caso coletivo, envolvendo entrevistas durante a internação do bebê e no 3º mês após a alta hospitalar. Os relatos dos participantes sobre a paternidade foram examinados através de análise qualitativa com base em quatro categorias derivadas da literatura e dos dados: representações acerca de si mesmo como pai, representações acerca da filha, relação pai-filha e representações acerca da família de origem. Os resultados revelaram o sofrimento dos pais relacionado à condição de vulnerabilidade do bebê nascido pré-termo e aos riscos envolvidos na situação de pré-eclâmpsia, particularmente entre os pais cujas filhas apresentaram menor peso e tempo gestacional. Os relatos sobre a gestação e parto também revelaram intensas ansiedades, com destaque para o sentimento de impotência e as preocupações referentes à saúde, sobrevivência e condição emocional da esposa. Após o parto, apesar dos temores sobre o futuro da criança, os participantes, em maior ou menor grau, mostraram-se bastante dedicados e atenciosos como pais, especialmente, durante os primeiros dias de vida de suas filhas, incluindo as visitas frequentes ao bebê, a relação pai-filha e o apoio emocional à esposa. Três meses após a alta do bebê, os relatos paternos evidenciaram o quanto as filhas, embora saudáveis, ainda eram percebidas como bebês bastante vulneráveis, desencadeando uma série de cuidados e, consequentemente, tendo importantes repercussões em suas vidas. Juntos, os achados do presente estudo sugerem a importância das maternidades, UTINeos e profissionais contemplarem a complexidade da situação do nascimento prematuro bem como as particularidades de cada caso. Além disso, destaca a necessidade de se oferecer intervenções psicológicas não só para as mães, mas também aos pais dos bebês nascidos pré-termo.pt_BR
dc.description.abstractThe present study aimed to investigate the transition to fatherhood in a context of preterm birth since the hospitalization in neonatal intensive care unit (NICU) up to three months after discharge. Retrospectively, this study also has the objective to investigate the father’s expectations and feelings during pregnancy. Three fathers of preterm born children, aged from 27 to 45, living with the babies’ mothers participated on this study. Two participants were first-time fathers, and one of them had two other children. The babies had different clinical characteristics at birth, including one extremely preterm infant with extremely-low-birth-weight, one very preterm infant with very low-weight and another one moderately preterm with low-birth-weight. A collective-case study design was developed involving fathers interviews during the baby hospital stay and in the third month post-discharge. The reports from participants about fatherhood were examined through qualitative analysis based on four categories derived from the literature and data: representations about himself-as-father, representations about the infant, father-infant relationship and representations on the family of origin. The results demonstrated the suffering of fathers related to the condition of the preterm infant vulnerability and the risks involved in the situation of pre-eclampsia, particularly among fathers whose infants had lower birth weight and shorter gestational time. Reports on pregnancy and childbirth also revealed intense anxieties, emphasizing helplessness feelings and concerns with the wife’s health, survival and emotional condition. After delivery, despite fears about the child's future, participants, to a greater or lesser degree, were very dedicated and caring as parents, especially during the first days of life of their infants, including frequent visits to the baby, the father-infant relationship and the emotional support to the wife. Three months after discharge, parental reports showed how the infants, although healthy, were still perceived as very vulnerable babies, triggering a series of care and, therefore, having important repercussions in their lives. Altogether, the findings of this study suggest the importance of contemplating the complexity of the situation of premature birth as the particularities of each case. Furthermore, the need of providing psychological interventions not only to mothers but also to child born preterm fathers should be emphasized.en
