dc.description.abstractOs objetivos do presente estudo foram: 1) avaliar o efeito sobre a variabilidade da excreção urinária de albumina dos fatores : manutenção de uma mesma dieta, quanto à composição e conteúdo calórico, período de coleta de urina e controle metabólico do diabete (estudo I) e 2) avaliar a resposta albuminúrica ao exercício físico máximo em indivíduos com diabete mélito insulino-dependente (estudo II). Do primeiro estudo participaram 32 pacientes com diabete mélito insulino-dependente, 18 homens e 14 mulheres, com idades de 12 a 48 anos (média 25.7 anos) e tempo de doença de 2 a 22 anos (média de 8.2 anos), sem hipertensão arterial sistêmica, infecção urinária, insuficiência cardíaca e cetonúria ou proteinúria. Como controles foram avaliados 13 indivíduos normais, cujas características físicas e idades eram semelhantes às do grupo diabético. Na mesma ocasião em coletado sangue para dosagem sérica de glicose plasmática de jejum, frutosamina, colesterol e triglicerídeos e urina de 24 horas, separada em ternos diurno e noturno para determinação de excreção urinária de albutruna e uréia urinária. Nos três dias do estudo os indivíduos eram orientados a ingerir uma dieta semelhante, o que foi comprovado posteriormente através da avaliação nutricional dos registros das dietas. Os resultados do primeiro estudo mostraram que não houve correlação entre a albuminúria noturna, diurna e de 24 horas com a ingestão calórica, proteica, lipídica e de carboidratos média, tanto em indivíduos normais como diabéticos. A dieta ingerida foi semelhante nos 3 dias de estudo nos dois grupos estudados. Observou-se que a excreção urinária de albumina determinada na urina coletada por 24 horas era, em média, 1.2 vezes maior do que aquela obtida no período noturno; quando determinada em urina diurna, era 1.3 vezes maior do que a noturna, além de que a albuminúria apresentava correlação positiva entre cada tipo de coleta utilizada. A excreção urin.ária de albumina noturna de indivíduos nonnoalbuminúricos foi de 6.4 ± 0.8 JJ.g/min e a de 24 horas foi de 6.57 ± 0.7 pg/min, maior do que a dos individues normais estudados (3.3 t 0.4 pg/m.in e 4.1 ± 0.6 pglm.in, respectivamente). O coeficiente de variação da excreção urinária de albumina noturna foi de 39 e 42%, da diurna de 40 e 39% e da de 24 horas de 28 e 32% para os indivíduos diabéticos e normais, respectivamente. Observou-se correlação positiva entre a glicose plasmática média e a excreção urinária de albumina noturna (r=0.32) e entre a frutosamina -nédia e a excreção urinária de albumina noturna ( r=0.43). No segundo estudo foram avaliados 13 pacientes com diabete mélito insulino-dependente, com idades de 16 a 32 anos (média de 22.3 anos), todos do sexo masculino, com tempo de diabete de 2 a 21 anos (média de 6.6. anos), sem microalbuminú.ria em repouso, semelhantes fisicamente e quanto à idade e sexo aos controles normais, em número de 5 Todos foram submetidos a mn teste de esforço máximo para determinação da albuminúria pós-exercício, após preparo com hidratação oral por aproximadamente 2 homs, com o objetivo de obter excreção urinária de albumina prévia ao exercício baixa ( <20 pg/m.in) e semelhante entre os indivíduos testados. As respostas metabólicas, cardiovasculares e físicas ao teste de esforço máximo foram semelhantes para os dois grupos estudados. A excreção urinária de albumina pré-exercicio foi de 6.5 ± 1.6 lJg/min no grupo diabético e de 6.9 ± 3.9 ug/min no grupo controle. A alburninúria determinada após o exercício físico foi de 161.2 ± 31.6 ~mínimo primeiro grupo c de 304.6 ± 130.8 Jl81'min no segundo. Estes resultados não foram estatisticamente diferentes entre os grupos. O coeficiente de variação da excreção urinária de albumina pós-exercício foi de 41%. Observou-se correlação positiva entre a excreção urinária de albumina obtida em amostras pré e pós-exercício (r de 0.53) e diurnas e de 24 horas e pós-exercício (r de 0.74 e 0.71 respectivamente). Os resultados obtidos nos dois estudos nos permitiram concluir que: 1) a excreção urinária de albumina medida em urina de 24 horas é maior do que a medida durante a noite, provavelmente devido a diferenças na atividade física e ou postura; 2) os pacientes com diabete mélito insulino-dependente normoolbuminúricos apresentam maiores valores de excreção urinária de albumina do que os indivíduos normais, o que não pode ser expliCAdo pela dieta ingerida habitualmente nem pelo exercício físico regular, mas sim por fatores reversiveis que causam aumento da albuminúria, como descontrole metabólico moderado ou pela presença, dentre estes pacientes, de indivíduos em fase de transição para microalbuminúria.; 3) 8 variabilidade da excreção urinária de albumina não deve estar relacionada 8 variações na ingestão alimentar, já que permanece elevada com a manutenção da dieta regular e 4) a coleta de urina para medida de albuminúria após exercício físico máximo não nos parece recurso útil no diagnóstico precoce da nefropatia diabética, já que, como as de repouso, apresenta alta variabilidade, além de refletir as mesmas alterações observadas em condições basais.pt_BR
dc.description.abstractThe purposes of the present study were: 1) to evaluate the effect on the variability of urinary albumin excretion of: maintainance of the same diet, concerning the oomposition and caloric content, time of urine collection and metabolic control of diabetes (study n and 2) to evaluate the albwninuric response to maximum physical exercise in insulin dependent diabetic individuais (study ll). In the first study, 32 insulin-dependent diabetic patients have participated, 18 men and 14 women, aged 12 to 48 years old (average 25.7) and diabetes duration from 2 to 22 years (average 8.2), without systemic hypertension, urinary tract infection, heart failure and ketonuria or proteinuria. As controls, 13 normal individuais were evaluated, whose physical characteristics and ages were similar to those of the diabetic group. Blood was collected for determination of fast plasmatic glucose, fructosamine, cholesterol and triglycerides, simultaneously with 24 hour urine collection, divided in day and night collection for urinary albumin excretion and urinary urea determination. During the 3 days of the study, the individuais were oriented to ingest a similar diet, which was confirmed afterwnrds, through nntricional evaluation of the diet records. The results of the first study showed that there was no correlation between nocturnal, diurna} and 24 hour albuminuria and the avemge intake of caloric energy, prateio, fat and carbohydrntes, in normais and diabetic individuais. The ingested diet was simular in the three days of study for both groups. It was observed that 24 hour urinary albumin excretion was 1.2 times greater than that collected through the night; when it was determined during the day, it was 1.3 times greater than that collected through the night. Besides that, albuminuria was positively correlated between each type of urine collection. The noctunal uriruuy album.in excretion of normoalbuminuric individuais was 6.4 ± 0.8 J.181min, and the 24 hour one was 6.57 ± 0.7 g/min, greater than that of the normal individuals studied (3.3 ± 0.4 J.181min and 4.1 -t 0.6 min , respectively). The coefficient of variation for nocturnal urinary albumin excretion was 39 and 42%, the diumal40 and 39% and the 24 hour 28 and 32% for normal and diabetic individuals, respectively. A positive correlation was observed between average fasting plasma gluoose and noctumal urinary albumin excretion (r=0.32) and between average fructosamine and noctumal urinary albumin excretion ( r= 0.4 3). In the second study, 13 insulin-dependent diabetic patients were evaluated, aged 16 to 32 years old (average 22.3), all male, with diabetes duration from 2 to 21 years (average 6.6), without microalbuminuria at rest, physically and with ages and gender distribution similar to 5 normal controls. Ali of them performed a maximal exercise test for determination of post-exercise albuminuria, after being prepared with oral hydration for 2 hours, with the purpose of obtaining a low urinary album.in excretion previous to the test ( < 20 J.lglmin) and similar between the individuals tested. The metabolic, cardiovascular and physical responses to the maximum exercise test were similar for both groups studied. The pre-exercise urinary albumin excretion was 6.5 ± 1.6 p.g/min in the diabetic group and 6.9 ± 3.9 J.lg/min in the control group. The albuminuria determined after physical exercise was 161.2 ± 31.6 J.lglmin in the first group and 304.6 ± 130.8 JJg/min in the second one. The results obtained in the two studies let us to conclude: 1) that urinary albumin excretion measured in 24 hour urine collection is greater than that measured over night, probably due to differences in physical activity and posture; 2) that the normoalbwninuric insulin-dependent diabetic patients have greater urinary albumin excretion than normal individuais, which cannot be explained by the usual ingested die~ neither by regular physical exercise, but by reversible factors that increase albuminuri.a, such as modera te metabolic discompensation or presence, among these patients, of individW!.ls in transition to a microalbuminuric phase; 3) the variability ofuriruuy albumin excretion should not be related to variation in diet intake, as it is elevated with the maintainance of the regular diet; 4) the urine collection for measurement of albuminuria after maximal exercise doesn't seem to be useful resource in the early diagnose of diabetic nephropaty, as, l.ike those obtained at rest, it has high variability, besides reflecting the same alterations observed in basal conditions.en
