dc.description.abstractEste trabalho teve por objetivo a caracterização das primeiras alimentações enterais dos recém-nascidos prematuros internados na Unidade de Terapia Intensiva Neonatal (UTIN) do Hospital de Clínicas de Porto Alegre (HCPA), que tiveram diagnóstico de Enterocolite Necrosante (ECN), no período de janeiro de 2007 a janeiro de 2010. A ECN é uma patologia que afeta, em 90% dos casos, bebês pré-termos. Conquanto, sua etiologia ainda é pouco conhecida pela ciência, dificultando medidas de prevenção. Seus sinais são, inicialmente, inespecíficos, sendo esta a causa principal de um diagnóstico tardio da doença. O tratamento é realizado de acordo com os estágios da ECN, caracterizados usualmente pelos estágios de Bell. Práticas alimentares podem influenciar no surgimento da ECN. O método utilizado foi o estudo transversal retrospectivo, onde, por meio dos prontuários, foram preenchidas fichas com dados da alimentação enteral destes bebês. Os resultados encontrados sobre a população foi uma média de 30,7 semanas de idade gestacional, peso ao nascer de 1279g, sendo 64,7% dos prematuros adequados para a idade gestacional (AIG), 35,3% dos bebês pequenos para idade gestacional (PIG) e 58,8% foram a óbito. O início da alimentação teve uma mediana de 3,5 dias de vida, volume inicial de 20 ml/kg/dia e velocidade inicial de 1 ml/h. Dos prematuros estudados, 17,6% iniciaram dieta enteral com leite materno (LM), 41,1% com LM mais fórmula láctea e 35,2% somente fórmula láctea. A progressão da dieta foi de 25 ml/kg/dia. No dia da confirmação do diagnóstico de ECN, o mesmo dia do raio-X de abdome, observaram-se as seguintes características: o diagnóstico se confirmou numa mediana de 6 dias de vida e 41,1% dos pré-termos apresentaram resíduo gástrico nesse dia. Quanto à alimentação, 52% já se encontravam em NPO no dia do diagnóstico, 12% recebiam leite materno e 36% recebiam LM mais a fórmula láctea. A primeira alimentação enteral trófica foi iniciada com tipos mistos de leite, e, somente, 17,6% dos bebês iniciaram com leite materno. Apenas pouco mais da metade da amostra foi exposta ao leite humano (58,4%). Alguns autores sugerem que o uso de LM exclusivo, nas primeiras alimentações enterais, é um dos fatores que pode prevenir o surgimento da ECN. A progressão da dieta se deu de maneira mais rápida que o recomendado, o que é um fator que pode influenciar no desencadeamento da ECN. Os dados do estudo são de importância para a prática de enfermagem em UTIN, porque evidenciam a prevalência das características da alimentação do pré-termo. Apesar das limitações deste trabalho, sugere-se o desenvolvimento de mais pesquisas sobre o tema, com o intuito de investigar os fatores associados ao desenvolvimento da ECN e a prática hospitalar.pt_BR
dc.description.abstractThis research aimed to characterize the first enteral feedings of preterm infants admitted to the ICU of HCPA, diagnosed of NEC in the period from January 2007 to January 2010. NEC is a serious disease that affects 90% of cases, preterm babies. Although, it’s etiology still unknown to science, interfering in preventive measures. His signs are initially generic, which is the main cause of a late diagnosis. The treatment is in accordance with the stages of NEC, usually characterized by Bell’s stages. Feeding practices may influence the onset of NEC. The method used was a retrospective cross-sectional study, enteral feeding’s data collection of these babies were performed by medical records. The findings on population was an average of 30.7 weeks gestational age, birth weight was 1279 g, 35.3% were characterized as small for gestational age (SGA), 64,7% adequate for gestational age (AGA) and 58.8% of deaths. On the supply can be said that the initiation of feeding was a median 3.5 days of life, its initial volume 20 ml / kg / day, its initial speed 1 ml/h, 17, 6% started their enteral feeding with breast milk (BM), 41.1 % BM with milk’s formula, milk’s formula only 35.2%.The progression of the diet was 25 ml/ kg / day. On confirmation of the diagnosis of NEC, the same day of the x-ray of the abdomen were observed the following characteristics: the diagnosis was confirmed a median of 6 days of life, 41,1% presented gastric residue on the day of diagnosis. The feeding presented in the x-ray’s day were: 52% were receiving only parenteral nutrition (PN), 12% received breast milk and 36% received formula milk over BM. The first trophic enteral feeding was started with mixed types of milk, and only 17.6% of the newborns received breastfed. Just over half t he sample was exposed to human milk (58.4%). Some authors suggest that the exclusive use of BM in the early enteral feeds, is one of the factors that could prevent the onset of NEC. The diet progression occurred more quickly than recommended, which is a factor that can influence the onset of NEC. The data's study is importance to the nursing practice in the NICU because of the characteristics which show the prevalence of feeding preterm infants. Despite the limitations of this work, we suggest the development of more research on the subject, in order to investigate the factors associated with the development of NEC and hospital practice.en
