dc.description.abstractRealizamos um estudo observacional de pacientes com mucopolissacaridose tipo VI, com o objetivo de determinar o perfil epidemiológico, clínico e bioquímico de um grupo de pacientes sul-americanos a fim de contribuir em estudos futuros de correlação genótipo-fenótipo e de avaliação de protocolos clínicos. Os critérios de inclusão foram: ter 4 anos ou mais e confirmação bioquímica da doença (níveis reduzidos da atividade da ARSB, aumento de GAGs urinários e atividade normal de outra sulfatase). Os critérios de exclusão foram: terapia com reposição enzimática atual ou prévia ou ter realizado transplante de medula óssea. Foram avaliados 28 pacientes por anamnese, exame físico, acocardiograma, eletrocardiograma, avaliação oftalmológica, medidas de glicosaminoglicanos urinários e da atividade da N-acetilgalactosamina-4-sulfatase em leucócitos. A amostra estudada tinha 92,9% de brasileiros, sendo 53,8% da região sudeste. No momento da avaliação, a média de idade foi de 97,1 meses e a média de idade ao diagnóstico foram de 48,4 meses. Em 88% da amostra os sintomas iniciaram com menos de 36 meses e em 27% das famílias houve relato de consangüinidade entre os pais. A média de peso e estatura ao nascimento foi de 3481 gramas e 51,3 centímetros, respectivamente. Da amostra, 57,1% nasceram de parto vaginal. Todos apresentavam alguma alteração ecocardiográfica, bem como opacificação corneana. As manifestações clínicas mais freqüentes foram: baixa estatura, opacificação corneana, facies grosseira, contraturas articulares e mãos em garra. A média da atividade enzimática em leucócitos foi de 5,4 nmoles/h x mg proteína e a excreção urinária de glicosaminoglicanos foi, em média, 7,9 vezes superior ao normal. O número de manifestações clínicas citadas não apresentou correlação significativa com a idade, com a excreção urinária de GAGs ou com a atividade enzimática em leucócitos. Também não houve correlação significativa entre a excreção urinária de GAGS e a atividade enzimática. Concluímos que a MPS VI é uma patologia com alta morbidade e que, comparados com a literatura, os pacientes da nossa amostra têm um diagnóstico tardio e maior freqüência de alterações cardiológicas.pt_BR
dc.description.abstractAn observational study of South-American patients with mucopolysaccharidosis type VI was carried out, aiming at determining their epidemiological, clinical and biochemical profile, and, to evaluate the correlation of signs/ symptoms reported with present age and biochemical findings (urinary excretion of GAGs and ARSB activity in lekocytes), so that it can serve as basis for future studies of the genotype-phenotype correlation and for protocols of future clinical studies. Each individual needed to meet the inclusion criteria: the signing of an informed term consent; age of four years or older; biochemical confirmation of the disease (reduction in level of ARSB activity and the increase in the levels of GAGs in urine, and a normal activity of another sulfatase). The exclusion criteria were: previous bone marrow transplant; previous or present enzyme replacement therapy. Twenty-eight patients were evaluated through anamnesis, physical examination, echocardiogram, electrocardiogram, ophthalmologic evaluation, and measurements of glycosaminoglycan levels in urine and of the activity of N-acetylgalactosamine-4-sulfatase (ARSB) in leukocytes. Of the sample studied, 92.9% were Brazilian, being 53.8% from the Brazil’s Southeast region. Mean age at evaluation was 97.1 months and at diagnosis 48.4 months. For 88% of patients, the onset of symptoms was before the age of 36 months, and in 27% of cases parental consanguinity was reported. Mean weight and height at birth were 3481 g and 51.3 cm, respectively. Birth was through vaginal delivery in 57.1% of cases. All patients presented some echocardiographic finding, as well as corneal clouding. The most frequently reported signs and symptoms were: short stature, corneal clouding, coarse facial features, joint contractures, and claw hands. Mean ARSB activity in leukocytes was 5,4 mmoles/h/mg prot. (rv: 72–174), and urinary excretion of glycosaminoglycans was, on average, 7.9 times higher than normal. The number of signs and symptoms reported has not shown a significant correlation with the age with the excretion of glycosaminoglycans nor with the level of enzyme activity in leukocytes. Also, no significant correlation was found between the excretion of glycosaminoglycans and the level of enzyme activity. It was concluded that MPS VI has high morbidity and that, when compared with data published in the literature, patients in our study had a later diagnosis and a higher frequency of cardiologic findings.en
