dc.description.abstractO NOD-like receptor family, pyrin domain containing 3 (NLRP3) é uma das proteínas mais bem caracterizadas do inflamassoma e é de particular interesse nas doenças autoimunes e inflamatórias. O complexo inflamassomal é responsável pela ativação das interleucinas (IL)-1β e IL-18. Na década de 90, polimorfismos envolvendo os genes responsáveis pela codificação de componentes do inflamassoma NLRP3 foram associados com um background sistêmico inflamatório. Desde então, as pesquisas envolvendo o complexo NLRP3 tem aumentado, assim como o leque de doenças associadas. Entre elas, doença de Crohn, infecção por HIV, diabetes tipo 1 e 2, resistência à insulina, lúpus eritematoso sistêmico, artrite, doença arterial coronariana, preeclampsia, doença celíaca e, recentemente, obesidade e rejeição aguda no transplante renal. O NLRP3 é altamente expresso em células epiteliais e células do sistema imune, como leucócitos, monócitos, granulócitos, células dendríticas, células B e células T. Estudos experimentais cada vez mais demonstram que a deficiência de NLRP3 previne a disfunção, o dano e a inflamação no rim em diversos modelos de doença renal crônica e aguda. O papel do inflamassoma na obesidade tem sido estudado extensivamente. O excesso de nutrientes resulta em um ambiente inflamatório, com níveis elevados de citocinas e quimiocinas, as quais estão envolvidas na defesa do hospedeiro e podem perpetuar o estado inflamatório através da indução de respostas imune inata. A expansão do tecido adiposo (TA) é o primeiro evento que levaria a inflamação. Células do sistema imune, incluindo macrófagos, infiltram o TA e ativam o NLRP3. A maioria dos estudos da literatura mostram modificações da expressão do NLRP3 no TA de pacientes com 14 obesidade. Entretanto, alguns resultados são controversos. Além disso, apenas um estudo avaliou a associação entre polimorfismos no gene NRLP3 e alteração de peso na literatura. Considerando que o ganho de peso é um desafio a ser enfrentado em pacientes transplantados renais e é associado à sobrevida do paciente e do aloenxerto, realizamos um estudo para avaliar se as alterações de peso pós-transplante poderiam estar associadas a variações em polimorfismos no gene NLRP3. Não há nenhum estudo avaliando a possível associação de polimorfismos neste gene e mudanças de peso pós-transplante renal até o momento. Sendo assim, em um estudo de coorte, polimorfismos no NLRP3 [(rs6672995 (G/A), rs4353135 (G/T) e rs10754558 (G/C)] foram genotipados em 449 receptores de transplante renal no Sul do Brasil. O alelo G (CG/GG) do polimorfismo rs10754558 foi significantemente associado com Δ peso (Beta = -1,385, IC 95% -2,763 – -0,007, P = 0,049), ajustado para idade do receptor. Entretanto, os polimorfismos rs6672995 e rs4353135 não foram associados com mudanças de peso pós-transplante. A rejeição aguda (RA) é um dos principais fatores deletérios do aloenxerto renal e pode levar à sua falha no primeiro ano após o transplante. Apesar de existir diversos estudos sobre o NLRP3 em doenças imunológicas e inflamatórias, apenas um estudo foi realizado em receptores de transplante renal e mostrou associação entre o polimorfismo rs6672995 no NRLP3 e a RA. Considerando o potencial papel desse gene como ferramenta de diagnóstico da RA, o objetivo do presente projeto foi verificar a associação entre os polimorfismos rs6672995 (G/A), rs4353135 (G/T) e rs10754558 (G/C) no gene NLRP3 e RA em receptores de transplante renal. Em um estudo caso-controle aninhado a uma coorte, os polimorfismos de interesse no gene NLRP3 foram genotipados em 449 receptores de transplante renal do Sul do Brasil, divididos em 98 indivíduos com RA e 351 15 indivíduos sem RA. Entretanto, os polimorfismos do NLRP3 não foram associados à RA nessa população. Em conclusão, nosso estudo investigou, pela primeira vez, a associação entre polimorfismos no NLRP3 e mudanças de peso pós-transplante. O alelo G do polimorfismo rs10754558 foi significativamente associado com diminuição do Δ peso (CG/GG: 1.69 ± 7.31 kg vs. CC: 3.09 ± 7.92 kg, P = 0.040). O alelo G (CG/GG) conferiu uma diminuição de 1,38 kg na variável Δ peso aos seus portadores, controlado para idade. Além disso, demonstramos que os polimorfismos no NLRP3 (rs6672995, rs4353135 e rs10754558) não parecem estar associados à RA em receptores renais.pt_BR
dc.description.abstractNOD-like receptor family, pyrin domain containing 3 (NLRP3) is one of the best characterized proteins of the inflammasome and is of particular interest in autoimmune and inflammatory diseases. The complex is responsible for the activation of interleukins (IL)-1β and IL-18. Two decades ago, polymorphisms involving the genes responsible for encoding the components of NLRP3 inflammasome were associated with a systemic inflammatory background. Since these findings, the research regarding NLRP3 complex has increased and also the bunch of related disease. The NLRP3 was associated with several diseases including crohn’s disease, HIV infection, type 1 and 2 diabetes, insulin resistance, systematic lupus erythematous, arthritis, coronary artery disease, preeclampsia, celiac disease, and recently, obesity and acute rejection in kidney transplantation. NLRP3 is highly expressed in kidney epithelial cells and immune system cells, such as leukocytes, monocytes, granulocytes, dendritic cells, T cells and B cells. Experimental studies increasingly show that NLRP3 deficiency prevents kidney dysfunction, damage and inflammation in various models of acute and chronic kidney disease. Studies have demonstrated the important role of the NLRP3 inflammasome in obesity. Nutrients excess can result in an inflammatory environment, with elevated levels of cytokines and chemokines, which are involved in host defense and inflammation by inducing innate immune inflammatory responses. Previous studies proposed that massive expansion of adipose tissue (AT) is the first event that causes inflammation. Cells of immune system, including macrophages, infiltrate the AT and activate the NLRP3 inflamassome. The majority of studies show modifications in NLRP3 expression in TA of patients with obesity. However, results are still controversial. In addition, only one study 17 evaluated an association between polymorphisms in the NRLP3 gene and weight change in the literature. Considering the weight gain has been a challenge in kidney transplant recipients and it was associated to patient’s survival rate and poorer graft, we performed a study to evaluate if weight changes post-transplantation could be associated with NLRP3 polymorphisms. There is no study evaluating the possible association of polymorphisms in this gene and weight changes post-transplantation until the moment. Thus, in a cohort study, the NLRP3 polymorphisms [(rs6672995 (G/A), rs4353135 (G/T) and rs10754558 (G/C)] were genotyped in 449 kidney transplant recipients from Southern Brazil. The G allele (CG/GG) of the rs10754558 polymorphism was significantly associated with Δ weight (Beta = -1.385, IC 95% -2.763 – -0.007, P = 0.049), adjusting for age of the recipient. However, rs6672995 and rs4353135 polymorphisms were not associated with weight changes. Acute rejection (AR) is one of the major deleterious factors of kidney graft and may lead to failure in the first year after transplantation. Despite the amount of studies regarding NLRP3 in immune and inflammatory diseases, only one study was performed in kidney transplant recipients, showing an association between the rs6672995 polymorphism in the NLRP3 gene and AR. Considering the potential role of this gene as diagnosis tool for AR, the aim of the study was to verify the association between the rs6672995 (G/A), rs4353135 (G/T) and rs10754558 (G/C) polymorphisms in the NLRP3 gene and AR in kidney transplant recipients. In a nested cohort case-control study, NLRP3 polymorphisms were genotyped in 449 kidney transplant recipients from Southern Brazil, divided into 98 individuals with AR and 351 individuals without AR. However, none of the NLRP3 polymorphisms were associated with AR in this population. In conclusion, our study investigated, for the first time, the association between NLRP3 polymorphisms and weight changes post-transplantation. The G allele of the rs10754558 polymorphism was significantly associated with decreased Δ weight (CG/GG: 1.69 ± 7.31 kg vs. CC: 3.09 ± 7.92 kg, P = 0.040). The G allele (CG/GG) conferred a decrease of 1.38 kg in the Δ weight of their carriers, controlling for age. Moreover, we demonstrated that NLRP3 polymorphisms (rs6672995, rs4353135 and rs10754558) seem not to be associated with AR in kidney recipients.en
